Informed consent is a principle in medical ethics , medical law , media studies , and other fields, that a person must have sufficient information and understanding before making decisions about accepting risk, such as their medical care. Pertinent information may include risks and benefits of treatments, alternative treatments, the patient's role in treatment, and their right to refuse treatment . In most systems, healthcare providers have a legal and ethical responsibility to ensure that a patient's consent is informed. This principle applies more broadly than healthcare intervention, for example to conduct research, to disclose a person's medical information, or to participate in high risk sporting and recreational activities.

134-706: Within the US, definitions of informed consent vary, and the standard required is generally determined by the state. These standards in medical contexts are formalized in the requirement for decision-making capacity and professional determinations in these contexts have legal authority. This requirement can be summarized in brief to presently include the following conditions, all of which must be met in order for one to qualify as possessing decision-making capacity: Impairments to reasoning and judgment that may preclude informed consent include intellectual or emotional immaturity, high levels of stress such as post-traumatic stress disorder or

268-561: A clinical trial , they are subject to review by an ethics committee or institutional review board . Informed consent is codified in both national and international law. 'Free consent' is a cognate term in the International Covenant on Civil and Political Rights , adopted in 1966 by the United Nations , and intended to be in force by 23 March 1976. Article 7 of the covenant prohibits experiments conducted without

402-412: A norepinephrine /cortisol ratio consequently higher than comparable non-diagnosed individuals. This is in contrast to the normative fight-or-flight response , in which both catecholamine and cortisol levels are elevated after exposure to a stressor. Brain catecholamine levels are high, and corticotropin-releasing factor (CRF) concentrations are high. Together, these findings suggest abnormality in

536-455: A case where such interference is likely, and after careful consideration, a researcher may forgo the informed consent process. This may be done after the researcher(s) and an Ethics Committee and/or Institutional Review Board (IRB) weigh the risk to study participants against the benefits to society and whether participants participate voluntarily and are to be treated fairly. The birth of new online media, such as social media, has complicated

670-780: A cause of tobacco use initiation in adolescents. AAP supports a minimum purchasing age of 21 years for tobacco products. Increasing age of purchase has been shown to decrease youth smoking rates. Younger age of starting tobacco use leads to lower rates of ever stopping tobacco use . According to Block (2023) , the AAP and other American medical professional groups have becoming increasingly aligned in supporting gender affirming care for gender dysphoria, which may include gonadotrophin releasing hormone analogues (GnRHa) to suppress puberty; oestrogen or testosterone to promote secondary sex characteristics; and surgical removal or augmentation of breasts, genitals, or other physical features. In 2018,

804-636: A child with chronic illnesses. Research exists which demonstrates that survivors of psychotic episodes , which exist in diseases such as schizophrenia , schizoaffective disorder , bipolar I disorder , and others, are at greater risk for PTSD due to the experiences one may have during and after psychosis. Such traumatic experiences include, but are not limited to, the treatment patients experience in psychiatric hospitals , police interactions due to psychotic behavior, suicidal behavior and attempts, social stigma and embarrassment due to behavior while in psychosis, frequent terrifying experiences due to psychosis, and

938-549: A common-law standard of inability to understand the nature of the procedure. In cases of incompetent adults, a health care proxy makes medical decisions. In the absence of a proxy, the medical practitioner is expected to act in the patient's best interests until a proxy can be found. By contrast, ' minors ' (which may be defined differently in different jurisdictions) are generally presumed incompetent to consent, but depending on their age and other factors may be required to provide Informed assent . In some jurisdictions (e.g. much of

1072-414: A doctor should lie to patients. In Hooker's view, benevolent deception is not fair to the patient, and he lectured widely on this topic. Hooker's ideas were not broadly influential. The US Canterbury v. Spence case established the principle of informed consent in US law. Earlier legal cases had created the underpinnings for informed consent, but his judgment gave a detailed and thought through discourse on

1206-496: A healthcare practitioner does not know which treatment is better in a randomized control trial can be harmful to the doctor-patient relationship. The doctrine of informed consent relates to professional negligence and establishes a breach of the duty of care owed to the patient (see duty of care , breach of the duty , and respect for persons ). The doctrine of informed consent also has significant implications for medical trials of medications, devices, or procedures. Until 2015 in

1340-537: A loved one accounts for approximately 20% of PTSD cases worldwide. Medical conditions associated with an increased risk of PTSD include cancer, heart attack, and stroke. 22% of cancer survivors present with lifelong PTSD like symptoms. Intensive-care unit (ICU) hospitalization is also a risk factor for PTSD. Some women experience PTSD from their experiences related to breast cancer and mastectomy . Loved ones of those who experience life-threatening illnesses are also at risk for developing PTSD, such as parents of

1474-495: A loved one is the most common traumatic event type reported in cross-national studies. However, the majority of people who experience this type of event will not develop PTSD. An analysis from the WHO World Mental Health Surveys found a 5.2% risk of developing PTSD after learning of the unexpected death of a loved one. Because of the high prevalence of this type of traumatic event, unexpected death of

1608-490: A maladaptive learning pathway to fear response through a hypersensitive, hyperreactive, and hyperresponsive HPA axis. Low cortisol levels may predispose individuals to PTSD: Following war trauma, Swedish soldiers serving in Bosnia and Herzegovina with low pre-service salivary cortisol levels had a higher risk of reacting with PTSD symptoms, following war trauma, than soldiers with normal pre-service levels. Because cortisol

1742-470: A marker of microglial activation ( 18-kDa translocator protein ) following lipopolysaccharide administration. This neuroimmune suppression is also associated with greater severity of anhedonic symptoms. Researchers suggest that treatments aimed at restoring neuroimmune function could be beneficial for alleviating PTSD symptoms. A meta-analysis of structural MRI studies found an association with reduced total brain volume, intracranial volume, and volumes of

1876-600: A mask without increased face touching. Teachers' unions opposed the AAP statement, however, saying "Our educators are overwhelmingly not comfortable returning to schools ... They fear for their lives, the lives of their students and the lives of their families." Two weeks later, the AAP walked back its support, under political pressure from teachers and other groups. The then-president Donald Trump cited AAP's original statement repeatedly, pressuring school leaders to reopen schools. In its 2020 statement in Pediatrics ,

2010-519: A mental health issue exists, it most often stems from stigma and negative experiences rather than being intrinsic to the child. The AAP also describe conversion therapy as "unsuccessful", "deleterious" and "outside the mainstream of traditional medical practice". Finally, the AAP recommends that youth identifying as transgender have access to comprehensive and development-appropriate healthcare provided in safe and inclusive clinics but also that family based therapy be available. The AAP also recommend that

2144-463: A non-life-threatening traffic accident, and a similar proportion of children develop PTSD. Risk of PTSD almost doubles to 4.6% for life-threatening auto accidents. Females were more likely to be diagnosed with PTSD following a road traffic accident , whether the accident occurred during childhood or adulthood. Post-traumatic stress reactions have been studied in children and adolescents. The rate of PTSD might be lower in children than adults, but in

2278-512: A note that exceptions could always be made when the doctor and family jointly agree to an older age. Recent studies have shown that the age of 21 years is just an arbitrary line for adolescence , because brain development does not fully reach adult levels of functioning until the early 30s. In a 2017 policy update, AAP changed its policy to discourage age limits of pediatric providers and instead have families reach an agreement with their pediatric provider as to when to transition care. In 2009,

2412-563: A particular clinical trial , after having been informed of all aspects of the trial that are relevant to the subject's decision to participate. Informed consent is documented by means of a written, signed, and dated informed consent form. In medical research , the Nuremberg Code set a base international standard in 1947, in response to the ethical violation in the Holocaust . Standards continued to develop. Nowadays, medical research

2546-543: A patient signed before onset of their illness. Brief examples of each of the above: For an individual to give valid informed consent, three components must be present: disclosure, capacity and voluntariness. As children often lack the decision-making ability or legal power (competence) to provide true informed consent for medical decisions, it often falls on parents or legal guardians to provide informed permission for medical decisions. This "consent by proxy" usually works reasonably well, but can lead to ethical dilemmas when

2680-779: A rear-facing car seat for as long as possible, until they meet the maximum height or weight dictated by the car seat manufacturer. On June 29, 2020, AAP stated that it "strongly advocates that all policy considerations for the coming school year should start with a goal of having students physically present in school" as remote learning during the COVID-19 pandemic in the United States made it more difficult for education professionals to notice learning deficits, physical and sexual abuse , depression and suicidal ideation . The AAP argued that masks are probably not practical for children younger than middle school unless they can wear

2814-416: A severe intellectual disability , severe mental disorder , intoxication , severe sleep deprivation , dementia , or coma . Obtaining informed consent is not always required . If an individual is considered unable to give informed consent, another person is generally authorized to give consent on the individual's behalf—for example, the parents or legal guardians of a child (though in this circumstance

2948-430: A smaller hippocampus might be more likely to develop PTSD following a traumatic event based on preliminary findings. Research has also found that PTSD shares many genetic influences common to other psychiatric disorders. Panic and generalized anxiety disorders and PTSD share 60% of the same genetic variance. Alcohol, nicotine, and drug dependence share greater than 40% genetic similarities. PTSD symptoms may result when

3082-531: A statewide research collaborative focused on transforming healthcare quality, health information systems and patient outcomes, developed an open-source system called Research Permissions Management System (RPMS). The ability to give informed consent is governed by a general requirement of competency. In common law jurisdictions, adults are presumed competent to consent. This presumption can be rebutted, for instance, in circumstances of mental illness or other incompetence. This may be prescribed in legislation or based on

3216-481: A substantial body of research has demonstrated that delaying the start of the school day is an effective countermeasure to chronic sleep loss and has a wide range of potential benefits to the physical and mental health, safety, and academic achievement of students—including reduced obesity risk, rates of depression, and drowsy driving crashes as well as improved academic performance and quality of life. Later start times also result in less frequent tardiness . There

3350-401: A traumatic event causes an over-reactive adrenaline response, which creates deep neurological patterns in the brain. These patterns can persist long after the event that triggered the fear, making an individual hyper-responsive to future fearful situations. During traumatic experiences, the high levels of stress hormones secreted suppress hypothalamic activity that may be a major factor toward

3484-444: A traumatic event in adulthood. It has been difficult to find consistently aspects of the events that predict, but peritraumatic dissociation has been a fairly consistent predictive indicator of the development of PTSD. Proximity to, duration of, and severity of the trauma make an impact. It has been speculated that interpersonal traumas cause more problems than impersonal ones, but this is controversial. The risk of developing PTSD

3618-747: A two-fold increased risk of death, with the leading causes of death being ischemic heart disease or cancers of the respiratory tract including lung cancer . Persons considered at risk for developing PTSD include combat military personnel, survivors of natural disasters, concentration camp survivors, and survivors of violent crime. Persons employed in occupations that expose them to violence (such as soldiers) or disasters (such as emergency service workers) are also at risk. Other occupations at an increased risk include police officers, firefighters, ambulance personnel, health care professionals, train drivers, divers, journalists, and sailors, as well as people who work at banks, post offices or in stores. The intensity of

3752-815: A way to encourage more participation. In 2023, the government of the United Kingdom proposed making this a requirement. The Ethical, Legal, and Social Implications research program of the National Human Genome Research Institute in the United States has provided some funding for researchers to do this. Other, long-standing controversies underscore the role for conflicts of interest among medical school faculty and researchers. For example, in 2014 coverage of University of California (UC) medical school faculty members has included news of ongoing corporate payments to researchers and practitioners from companies that market and produce

3886-634: A weekly report on COVID-19 cases in the United States . States began reporting COVID-19 cases on September 17, 2020. The AAP tracked 587,948 child COVID-19 cases, 5,016 child hospitalizations, and 109 child deaths. In 2006, the Academy received a grant from the Flight Attendant Medical Research Institute (FAMRI) to plan and establish a Center of Excellence dedicated to the elimination of children's exposure to tobacco and secondhand smoke . The Richmond Center

4020-597: A wide range of traumatic events. The risk of developing PTSD after a traumatic event varies by trauma type and is the highest following exposure to sexual violence (11.4%), particularly rape (19.0%). Men are more likely to experience a traumatic event (of any type), but women are more likely to experience the kind of high-impact traumatic event that can lead to PTSD, such as interpersonal violence and sexual assault . Motor vehicle collision survivors, both children and adults, are at an increased risk of PTSD. Globally, about 2.6% of adults are diagnosed with PTSD following

4154-440: Is a mental and behavioral disorder that develops from experiencing a traumatic event, such as sexual assault , warfare , traffic collisions , child abuse , domestic violence , or other threats on a person's life or well-being. Symptoms may include disturbing thoughts, feelings, or dreams related to the events, mental or physical distress to trauma -related cues, attempts to avoid trauma-related cues, alterations in

4288-468: Is a basic right and should be carried out effectively, if a patient is incapacitated due to injury or illness, it is still important that patients benefit from emergency experimentation. The Food and Drug Administration (FDA) and the Department of Health and Human Services (DHHS) joined to create federal guidelines to permit emergency research, without informed consent. However, they can only proceed with

4422-510: Is also associated with PTSD. There is evidence that susceptibility to PTSD is hereditary . Approximately 30% of the variance in PTSD is caused from genetics alone. For twin pairs exposed to combat in Vietnam, having a monozygotic (identical) twin with PTSD was associated with an increased risk of the co-twin's having PTSD compared to twins that were dizygotic (non-identical twins). Women with

4556-501: Is also higher if people around the survivor ignore (or are ignorant of) the rape or blame the rape survivor. Military service in combat is a risk factor for developing PTSD. Around 22% of people exposed to combat develop PTSD; in about 25% of military personnel who develop PTSD, its appearance is delayed. Refugees are also at an increased risk for PTSD due to their exposure to war, hardships, and traumatic events. The rates for PTSD within refugee populations range from 4% to 86%. While

4690-426: Is always a degree to which informed consent must be assumed or inferred based upon observation, or knowledge, or legal reliance. This especially is the case in sexual or relational issues. In medical or formal circumstances, explicit agreement by means of signature—normally relied on legally—regardless of actual consent, is the norm. This is the case with certain procedures, such as a " do not resuscitate " directive that

4824-581: Is an important public health issue that significantly affects the health and safety, as well as academic success , the American Academy of Pediatrics strongly supports efforts of school districts to optimize sleep in students and urges high schools and middle schools to aim for start times no earlier than 8:30 a.m., to allow students the opportunity to achieve optimal levels of sleep (8.5–9.5 hours) and to improve physical and mental health, safety, academic performance, and quality of life. Although

4958-810: Is common to have symptoms after any traumatic event, these must persist to a sufficient degree (i.e., causing dysfunction in life or clinical levels of distress) for longer than one month after the trauma to be classified as PTSD (clinically significant dysfunction or distress for less than one month after the trauma may be acute stress disorder ). Some following a traumatic event experience post-traumatic growth . Trauma survivors often develop depression, anxiety disorders, and mood disorders in addition to PTSD. More than 50% of those with PTSD have co-morbid anxiety , mood or substance use disorders . Substance use disorder , such as alcohol use disorder , commonly co-occur with PTSD. Recovery from post-traumatic stress disorder or other anxiety disorders may be hindered, or

5092-520: Is controversial given the requirement for informed consent. Deception typically arises in social psychology, when researching a particular psychological process requires that investigators deceive subjects. For example, in the Milgram experiment , researchers wanted to determine the willingness of participants to obey authority figures despite their personal conscientious objections. They had authority figures demand that participants deliver what they thought

5226-493: Is gathered about the risks and benefits of the procedure. Electronic nicotine delivery systems (electronic cigarettes, e-hookahs, vape pens, others) are highly addictive and often candy-flavored products that are rapidly rising in popularity among middle and high school students, and appear to be serving as a gateway to other forms of tobacco, and threaten to addict a new generation to nicotine . The American Academy of Pediatrics says that although firearms-related deaths in

5360-404: Is increased in individuals who are exposed to physical abuse , physical assault , or kidnapping . Women who experience physical violence are more likely to develop PTSD than men. An individual that has been exposed to domestic violence is predisposed to the development of PTSD. There is a strong association between the development of PTSD in mothers that experienced domestic violence during

5494-572: Is internally digested. Consent may be implied within the usual subtleties of human communication, rather than explicitly negotiated verbally or in writing. For example, if a doctor asks a patient to take their blood pressure, a patient may demonstrate consent by offering their arm for a blood pressure cuff. In some cases consent cannot legally be possible, even if the person protests he does indeed understand and wish. There are also structured instruments for evaluating capacity to give informed consent, although no ideal instrument presently exists. Thus, there

5628-490: Is limited medical controversy surrounded the AAP regarding abusive head trauma (AHT), also called shaken baby syndrome in infants. The current skepticism is not whether violent shaking or shaking with slamming is dangerous to infants and children, but of how the scientific information is used in the legal processes. The AAP updated its policy paper in 2020. In the updated policy paper, the APP states "The AAP continues to affirm

5762-438: Is marked with an X. Tearoom Trade is the name of a book by American psychologist Laud Humphreys . In it he describes his research into male homosexual acts. In conducting this research he never sought consent from his research subjects and other researchers raised concerns that he violated the right to privacy for research participants . Post-traumatic stress disorder Post-traumatic stress disorder ( PTSD )

5896-533: Is normally important in restoring homeostasis after the stress response, it is thought that trauma survivors with low cortisol experience a poorly contained—that is, longer and more distressing—response, setting the stage for PTSD. It is thought that the locus coeruleus-noradrenergic system mediates the over-consolidation of fear memory. High levels of cortisol reduce noradrenergic activity, and because people with PTSD tend to have reduced levels of cortisol, it has been proposed that individuals with PTSD cannot regulate

6030-401: Is not mentioned. The Hippocratic Corpus advises that physicians conceal most information from patients to give the patients the best care. The rationale is a beneficence model for care—the doctor knows better than the patient, and therefore should direct the patient's care, because the patient is not likely to have better ideas than the doctor. Henri de Mondeville , a French surgeon who in

6164-581: Is overseen by an ethics committee that also oversees the informed consent process. As the medical guidelines established in the Nuremberg Code were imported into the ethical guidelines for the social sciences , informed consent became a common part of the research procedure. However, while informed consent is the default in medical settings, it is not always required in the social sciences. Here, firstly, research often involves low or no risk for participants, unlike in many medical experiments. Secondly,

6298-545: Is signed by both patient and clinician. In a number of healthcare organisations consent forms are scanned and maintained in an electronic document store. The paper consent process has been demonstrated to be associated with significant errors of omission, and therefore increasing numbers of organisations are using digital consent applications where the risk of errors can be minimised, a patient's decision making and comprehension can be supported by additional lay-friendly and accessible information, consent can be completed remotely, and

6432-423: Is similar to PTSD, but has a distinct effect on a person's emotional regulation and core identity. Prevention may be possible when counselling is targeted at those with early symptoms, but is not effective when provided to all trauma-exposed individuals regardless of whether symptoms are present. The main treatments for people with PTSD are counselling (psychotherapy) and medication. Antidepressants of

6566-440: Is to do research on infants and young children. When addressing the issue of informed consent with children, the primary response is parental consent. This is valid, although only legal guardians are able to consent for a child, not adult siblings. Additionally, parents may not order the termination of a treatment that is required to keep a child alive, even if they feel it is in the best interest. Guardians are typically involved in

6700-410: Is to give each statement a five-year life, after which the statement expires unless it is reaffirmed. The AAP is supportive of abortion rights and criticized the overturning of Roe v. Wade . In June 2022, they stated that adolescents "should have the right to receive legal and confidential medical and surgical abortion care and counseling" and that "[ Dobbs v. Jackson ] means that in many places in

6834-572: The American Medical Association was founded they in 1847 produced a work called the first edition of the American Medical Association Code of Medical Ethics . A new concept in this book was the idea that physicians should fully disclose all patient details truthfully when talking to other physicians, but

6968-515: The American Psychological Association says that psychologists may conduct research that includes a deceptive compartment only if they can both justify the act by the value and importance of the study's results and show they could not obtain the results by some other way. Moreover, the research should bear no potential harm to the subject as an outcome of deception, either physical pain or emotional distress . Finally,

7102-468: The SSRI or SNRI type are the first-line medications used for PTSD and are moderately beneficial for about half of people. Benefits from medication are less than those seen with counselling. It is not known whether using medications and counselling together has greater benefit than either method separately. Medications, other than some SSRIs or SNRIs, do not have enough evidence to support their use and, in

7236-648: The United Kingdom and in countries such as Malaysia and Singapore , informed consent in medical procedures requires proof as to the standard of care to expect as a recognised standard of acceptable professional practice (the Bolam Test ), that is, what risks would a medical professional usually disclose in the circumstances (see Loss of right in English law ). Arguably, this is "sufficient consent" rather than "informed consent." The UK has since departed from

7370-526: The hippocampus , insula cortex , and anterior cingulate . Much of this research stems from PTSD in those exposed to the Vietnam War. People with PTSD have decreased brain activity in the dorsal and rostral anterior cingulate cortices and the ventromedial prefrontal cortex , areas linked to the experience and regulation of emotion. The amygdala is strongly involved in forming emotional memories, especially fear-related memories. During high stress,

7504-421: The hippocampus , which is associated with placing memories in the correct context of space and time and memory recall, is suppressed. According to one theory, this suppression may be the cause of the flashbacks that can affect people with PTSD. When someone with PTSD undergoes stimuli similar to the traumatic event, the body perceives the event as occurring again because the memory was never properly recorded in

7638-523: The hypothalamic-pituitary-adrenal (HPA) axis . The maintenance of fear has been shown to include the HPA axis, the locus coeruleus - noradrenergic systems, and the connections between the limbic system and frontal cortex . The HPA axis that coordinates the hormonal response to stress, which activates the LC-noradrenergic system, is implicated in the over-consolidation of memories that occurs in

7772-432: The perinatal period of their pregnancy. Those who have experienced sexual assault or rape may develop symptoms of PTSD. The likelihood of sustained symptoms of PTSD is higher if the rapist confined or restrained the person, if the person being raped believed the rapist would kill them, the person who was raped was very young or very old, and if the rapist was someone they knew. The likelihood of sustained severe symptoms

7906-508: The thyroid hormone triiodothyronine in PTSD. This kind of type 2 allostatic adaptation may contribute to increased sensitivity to catecholamines and other stress mediators. Hyperresponsiveness in the norepinephrine system can also be caused by continued exposure to high stress. Overactivation of norepinephrine receptors in the prefrontal cortex can be connected to the flashbacks and nightmares frequently experienced by those with PTSD. A decrease in other norepinephrine functions (awareness of

8040-422: The "free consent to medical or scientific experimentation" of the subject. As of September 2019, the covenant has 173 parties and six more signatories without ratification. Informed consent can be complex to evaluate, because neither expressions of consent, nor expressions of understanding of implications, necessarily mean that full adult consent was in fact given, nor that full comprehension of relevant issues

8174-505: The 14th century, wrote about medical practice. He traced his ideas to the Hippocratic Oath. Among his recommendations were that doctors "promise a cure to every patient" in hopes that the good prognosis would inspire a good outcome to treatment. Mondeville never mentioned getting consent, but did emphasize the need for the patient to have confidence in the doctor. He also advised that when deciding therapeutically unimportant details

8442-422: The AAP acknowledges that numerous factors may impair the amount and/or quality of sleep in adolescents—among them, biological changes in sleep associated with puberty , lifestyle choices, and academic demands—it considers school start times before 8:30 a.m. ("earlier school start times") to be a key modifiable contributor to insufficient sleep, together with circadian rhythm disruption. It also recognizes that

8576-450: The AAP called for banning all digital advertising that was targeted to children under the age of 7 and urged limits to advertising aimed at people under 17. This aimed to protect kids from digital tracking on social media, Tv and video games. In a 2012 position statement, the academy stated that a systematic evaluation of the medical literature shows that the "preventive health benefits of elective circumcision of male newborns outweigh

8710-510: The AAP issued a policy statement putting forward a model of gender affirmative care. Gender affirmative care is based in the idea that transgender identities and diverse gender expressions do not constitute a mental disorder, that variations in gender identity and expression are normal aspects of human diversity, and binary definitions of gender do not always reflect emerging gender identities, that gender identity evolves as an interplay of biology, development, socialization, and culture, and that if

8844-471: The Academy recommends that pediatricians and regulators treat it as they would tobacco. The Academy supports "decriminalization" of marijuana (reductions in the penalties for its use and possession) in combination with an increased commitment to substance-abuse treatment . The Academy recommends changing marijuana from a DEA Schedule I to a DEA Schedule II to facilitate research into pharmaceutical uses . Recognizing that insufficient sleep in adolescents

8978-921: The Bolam test for judging standards of informed consent, due to the landmark ruling in Montgomery v Lanarkshire Health Board . This moves away from the concept of a reasonable physician and instead uses the standard of a reasonable patient , and what risks an individual would attach significance to. Medicine in the United States, Australia, and Canada takes this patient-centric approach to "informed consent." Informed consent in these jurisdictions requires healthcare providers to disclose significant risks, as well as risks of particular importance to that patient. This approach combines an objective (a hypothetical reasonable patient) and subjective (this particular patient) approach. Optimal establishment of an informed consent requires adaptation to cultural or other individual factors of

9112-1014: The Diagnostic and Statistical Manual, Fourth Edition (DSM-IV), revealed that 22% of cancer survivors present with lifetime cancer-related PTSD (CR-PTSD), endorsing cancer diagnosis and treatment as a traumatic stressor. Therefore, as the number of people diagnosed with cancer increases and cancer survivorship improves, cancer-related PTSD becomes a more prominent issue, and thus, providing for cancer patients' physical and psychological needs becomes increasingly important. Evidence‐based treatments such as eye movement desensitization and reprocessing (EMDR) therapy and cognitive-behavioral therapy (CBT) are available for PTSD, and indeed, there have been promising reports of their effectiveness in cancer patients. Women who experience miscarriage are at risk of PTSD. Those who experience subsequent miscarriages have an increased risk of PTSD compared to those experiencing only one. PTSD can also occur after childbirth and

9246-621: The Facebook News Feeds of roughly 700,000 users to reduce either the amount of positive or negative posts they saw for a week. The study then analyzed if the users' status updates changed during the different conditions. The study was published in the Proceedings of the National Academy of Sciences . The lack of informed consent led to outrage among many researchers and users. Many believed that by potentially altering

9380-480: The HPA axis by dexamethasone . Studies on the peripheral immune have found dysfunction with elevated cytokine levels and a higher risk of immune-related chronic diseases among individuals with PTSD. Neuroimmune dysfunction has also been found in PTSD, raising the possibility of a suppressed central immune response due to reduced activity of microglia in the brain in response to immune challenges. Individuals with PTSD, compared to controls, have lower increase in

9514-476: The U.S.), this is a strict standard. In other jurisdictions (e.g. England, Australia, Canada), this presumption may be rebutted through proof that the minor is 'mature' (the ' Gillick standard '). In cases of incompetent minors, informed consent is usually required from the parent (rather than the 'best interests standard') although a parens patriae order may apply, allowing the court to dispense with parental consent in cases of refusal. Research involving deception

9648-568: The US have dropped since the 1990s, guns were used in more than 80 percent of teen homicides in 2009 and were the most common suicide method among US teens. The AAP believes pediatricians should discuss guns and gun safety with parents before babies are born and at children's annual exams. It also advocates for, among other things, more background checks , an assault weapons ban , and more federal research on gun violence . The AAP warns of possible marijuana damage to children and adolescents. In states that have already legalized marijuana ,

9782-435: The United States, this evidence-based care will be difficult or impossible to access, threatening the health and safety of our patients and jeopardizing the patient-physician relationship". The AAP has changed positions on its age limit throughout the years. In 1988, the American Academy of Pediatrics published a statement on the age limit of pediatrics that identified the upper age limit of pediatrics as age 21. The policy had

9916-769: The absence of therapy, symptoms may continue for decades. One estimate suggests that the proportion of children and adolescents having PTSD in a non-wartorn population in a developed country may be 1% compared to 1.5% to 3% of adults. On average, 16% of children exposed to a traumatic event develop PTSD, with the incidence varying according to type of exposure and gender. Similar to the adult population, risk factors for PTSD in children include: female gender , exposure to disasters (natural or man-made), negative coping behaviors, and/or lacking proper social support systems. Predictor models have consistently found that childhood trauma, chronic adversity, neurobiological differences, and familial stressors are associated with risk for PTSD after

10050-610: The aftermath of trauma. This over-consolidation increases the likelihood of one's developing PTSD. The amygdala is responsible for threat detection and the conditioned and unconditioned fear responses that are carried out as a response to a threat. The HPA axis is responsible for coordinating the hormonal response to stress. Given the strong cortisol suppression to dexamethasone in PTSD, HPA axis abnormalities are likely predicated on strong negative feedback inhibition of cortisol, itself likely due to an increased sensitivity of glucocorticoid receptors . PTSD has been hypothesized to be

10184-446: The case of benzodiazepines , may worsen outcomes. In the United States, about 3.5% of adults have PTSD in a given year, and 9% of people develop it at some point in their life. In much of the rest of the world, rates during a given year are between 0.5% and 1%. Higher rates may occur in regions of armed conflict . It is more common in women than men. Symptoms of trauma-related mental disorders have been documented since at least

10318-449: The child may be required to provide informed assent ) and conservators for the mentally disordered . Alternatively, the doctrine of implied consent permits treatment in limited cases, for example when an unconscious person will die without immediate intervention. Cases in which an individual is provided insufficient information to form a reasoned decision raise serious ethical issues. When these issues occur, or are anticipated to occur, in

10452-492: The code requires a debriefing session in which the experimenter both tells the subject about the deception and gives subject the option of withdrawing the data. In some U.S. states, informed consent laws (sometimes called "right to know" laws) require that a woman seeking an elective abortion receive information from the abortion provider about her legal rights, alternatives to abortion (such as adoption ), available public and private assistance, and other information specified in

10586-501: The commonly associated behavioral symptoms such as anxiety, ruminations, irritability, aggression, suicidality, and impulsivity. Serotonin also contributes to the stabilization of glucocorticoid production. Dopamine levels in a person with PTSD can contribute to symptoms: low levels can contribute to anhedonia , apathy , impaired attention , and motor deficits; high levels can contribute to psychosis , agitation , and restlessness. hasral studies described elevated concentrations of

10720-550: The condition worsened, when substance use disorders are comorbid with PTSD. Resolving these problems can bring about improvement in an individual's mental health status and anxiety levels. PTSD has a strong association with tinnitus , and can even possibly be the tinnitus' cause. In children and adolescents, there is a strong association between emotional regulation difficulties (e.g. mood swings, anger outbursts, temper tantrums ) and post-traumatic stress symptoms, independent of age, gender, or type of trauma. Moral injury ,

10854-488: The consent of children, however a number of doctrines have developed that allow children to receive health treatments without parental consent. For example, emancipated minors may consent to medical treatment, and minors can also consent in an emergency. Waiver of the consent requirement may be applied in certain circumstances where no foreseeable harm is expected to result from the study or when permitted by law, federal regulations, or if an ethical review committee has approved

10988-573: The current environment) prevents the memory mechanisms in the brain from processing the experience, and emotions the person is experiencing during a flashback are not associated with the current environment. There is considerable controversy within the medical community regarding the neurobiology of PTSD. A 2012 review showed no clear relationship between cortisol levels and PTSD. The majority of reports indicate people with PTSD have elevated levels of corticotropin-releasing hormone , lower basal cortisol levels, and enhanced negative feedback suppression of

11122-691: The dangers and harms of shaking infants, continues to embrace the 'shaken baby syndrome' diagnosis as a valid subset of the AHT diagnosis, and encourages pediatric practitioners to educate community stakeholders when necessary." In 2008, the AAP and the American Heart Association recommended statins for children as young as eight years with high lipid concentrations and for children as young as two years with major cardiovascular risk factors, if weight management , dietary changes, and additional physical exercise were not sufficient to reduce

11256-422: The development of PTSD. PTSD causes biochemical changes in the brain and body, that differ from other psychiatric disorders such as major depression . Individuals diagnosed with PTSD respond more strongly to a dexamethasone suppression test than individuals diagnosed with clinical depression . Most people with PTSD show a low secretion of cortisol and high secretion of catecholamines in urine , with

11390-418: The differences in the ethical review process between publicly and privately funded research. Some say Facebook was within its limits and others see the need for more informed consent and/or the establishment of in-house private review boards. Some researchers and ethicists advocate for researchers to share experimental results with their subjects in a way they can understand, both as an ethical obligation and as

11524-486: The doctor should meet the patients' requests "so far as they do not interfere with treatment". In Ottoman Empire records there exists an agreement from 1539 in which negotiates details of a surgery, including fee and a commitment not to sue in case of death. This is the oldest identified written document in which a patient acknowledges risk of medical treatment and writes to express their willingness to proceed. Benjamin Rush

11658-480: The doctor was judged by the General Medical Council to have acted negligently. The council stated that the woman should have been informed of her condition, and allowed to make her own decision. To document that informed consent has been given for a procedure, healthcare organisations have traditionally used paper-based consent forms on which the procedure and its risks and benefits are noted, and

11792-435: The duties of patients to their physicians", he stated that patients should be strictly obedient to the physician's orders; this was representative of much of his writings. John Gregory, Rush's teacher, wrote similar views that a doctor could best practice beneficence by making decisions for the patients without their consent. Thomas Percival was a British physician who published a book called Medical Ethics in 1803. Percival

11926-540: The fear of losing control or actual loss of control. The incidence of PTSD in survivors of psychosis may be as low as 11% and as high at 67%. Prevalence estimates of cancer‐related PTSD range between 7% and 14%, with an additional 10% to 20% of patients experiencing subsyndromal posttraumatic stress symptoms (ie, PTSS). Both PTSD and PTSS have been associated with increased distress and impaired quality of life, and have been reported in newly diagnosed patients as well as in long‐term survivors. The PTSD Field Trials for

12060-409: The feeling of moral distress such as a shame or guilt following a moral transgression, is associated with PTSD but is distinguished from it. Moral injury is associated with shame and guilt, while PTSD is associated with anxiety and fear. In a population based study examining veterans of the Vietnam War , the presence of PTSD and exposure to high level stressors on the battlefield were associated with

12194-428: The fetus," but those critics acknowledge that "most of the information in the [legally mandated] materials about abortion comports with recent scientific findings and the principles of informed consent", although "some content is either misleading or altogether incorrect." Informed consent is part of ethical clinical research as well, in which a human subject voluntarily confirms his or her willingness to participate in

12328-409: The history of checking for any of these practices: These practices are part of what constitutes informed consent, and their history is the history of informed consent. They combine to form the modern concept of informed consent—which rose in response to particular incidents in modern research. Whereas various cultures in various places practiced informed consent, the modern concept of informed consent

12462-479: The human subject." Medical sociologists have studied informed consent as well bioethics more generally. Oonagh Corrigan, looking at informed consent for research in patients, argues that much of the conceptualization of informed consent comes from research ethics and bioethics with a focus on patient autonomy, and notes that this aligns with a neoliberal worldview. Corrigan argues that a model based solely around individual decision making does not accurately describe

12596-413: The idea of informed consent. In an online environment people pay little attention to Terms of Use agreements and can subject themselves to research without thorough knowledge. This issue came to the public light following a study conducted by Facebook in 2014, and published by that company and Cornell University . Facebook conducted a study without consulting an Ethics Committee or IRB where they altered

12730-568: The increased noradrenergic response to traumatic stress. Intrusive memories and conditioned fear responses are thought to be a result of the response to associated triggers. Neuropeptide Y (NPY) has been reported to reduce the release of norepinephrine and has been demonstrated to have anxiolytic properties in animal models. Studies have shown people with PTSD demonstrate reduced levels of NPY, possibly indicating their increased anxiety levels. Other studies indicate that people with PTSD have chronically low levels of serotonin , which contributes to

12864-426: The judgment of the parents or guardians and the medical professional differ with regard to what constitutes appropriate decisions "in the best interest of the child". Children who are legally emancipated , and certain situations such as decisions regarding sexually transmitted diseases or pregnancy, or for unemancipated minors who are deemed to have medical decision making capacity, may be able to provide consent without

12998-470: The law, before the abortion is performed. Other countries with such laws (e.g. Germany ) require that the information giver be properly certified to make sure that no abortion is carried out for the financial gain of the abortion provider and to ensure that the decision to have an abortion is not swayed by any form of incentive. Some informed consent laws have been criticized for allegedly using "loaded language in an apparently deliberate attempt to 'personify'

13132-517: The matter. The judgment cites cases going back to 1914 as precedent for informed consent. Historians cite a series of human subject research experiments to trace the history of informed consent in research. The U.S. Army Yellow Fever Commission "is considered the first research group in history to use consent forms." In 1900, Major Walter Reed was appointed head of the four man U.S. Army Yellow Fever Commission in Cuba that determined mosquitoes were

13266-420: The medical field and federal government prioritize research that is dedicated to improving the quality of evidence-based care for transgender youth. Brush, Book, Bed (BBB) is an American Academy of Pediatrics program aimed at improving children's health by creating a nighttime routine of brushing the child's teeth, reading together, and setting a bedtime . It was developed to help pediatricians communicate

13400-473: The mere knowledge that they participate in a study can cause people to alter their behavior, as in the Hawthorne Effect : "In the typical lab experiment, subjects enter an environment in which they are keenly aware that their behavior is being monitored, recorded, and subsequently scrutinized." In such cases, seeking informed consent directly interferes with the ability to conduct the research, because

13534-430: The mood of users by altering what posts they see, Facebook put at-risk individuals at higher dangers for depression and suicide. However, supporters of Facebook claim that Facebook details that they have the right to use information for research in their terms of use. Others say the experiment is just a part of Facebook's current work, which alters News Feeds algorithms continually to keep people interested and coming back to

13668-569: The more arduous tasks of cutting back on junk foods, promoting healthy diets, and putting physical education back into the schools." AAP recommends that tobacco control programs should change the image of tobacco by telling the truth about the substance. This includes prohibiting tobacco advertising and promotion that is accessible to children, as well as point of sale advertising, product placements in movies and other entertainment media, and promotion in print or internet-based media accessible to youth. Advertising and promotion has been shown to be

13802-592: The most reliable surgical devices and medication…and they shouldn't be treated as subjects in expensive experiments." Other UC incidents include taking the eggs of women for implantation into other women without consent and injecting live bacteria into human brains, resulting in potentially premature deaths. Informed consent is a technical term first used by attorney, Paul G. Gebhard, in the Salgo v. Leland Stanford Jr. University Board of Trustees court case in 1957. In tracing its history, some scholars have suggested tracing

13936-694: The national office and four of its State chapters provided training support to 49 pediatric practices to improve adherence to well-established asthma care guidelines. The percentage of patients at participating practices with well-controlled asthma (as defined by the National Heart, Lung, and Blood Institute ) rose from 58 to 72 percent. The AAP periodically issues guidance for child passenger safety , including policy recommendations for transitioning between rear-facing car seats , front-facing car seats, belt-positioning booster seats, and vehicle safety belts . These recommendations are typically published in

14070-470: The need for parental permission depending on the laws of the jurisdiction the child lives in. The American Academy of Pediatrics encourages medical professionals also to seek the assent of older children and adolescents by providing age appropriate information to these children to help empower them in the decision-making process. Research on children has benefited society in many ways. The only effective way to establish normal patterns of growth and metabolism

14204-637: The non-disclosure of certain information. Besides studies with minimal risk, waivers of consent may be obtained in a military setting. According to 10 USC 980, the United States Code for the Armed Forces, Limitations on the Use of Humans as Experimental Subjects, a waiver of advanced informed consent may be granted by the Secretary of Defense if a research project would: While informed consent

14338-496: The patient. As of 2011, for example, people from Mediterranean and Arab baqckgrounds appeared to rely more on the context of the delivery of the information, with the information being carried more by who is saying it and where, when, and how it is being said, rather than what is said, which is of relatively more importance in typical "Western" countries. The informed consent doctrine is generally implemented through good healthcare practice: pre-operation discussions with patients and

14472-411: The peer-reviewed scientific journal Pediatrics . Previously, the AAP recommended that children remain rear-facing until they are two years of age. In response to updated crash test, simulation, and field data, the AAP revised their guidance to exclude the age guideline entirely. Current AAP Child Passenger Safety recommendations (as of August 30, 2018 ) state that children should remain in

14606-524: The person's memory. American Academy of Pediatrics The American Academy of Pediatrics ( AAP ) is the largest professional association of pediatricians in the United States. It is headquartered in Itasca, Illinois , and maintains an office in Washington, D.C. The AAP has published hundreds of policy statements, ranging from advocacy issues to practice recommendations. The Academy

14740-454: The process can become paperless. One form of digital consent is dynamic consent , which invites participants to provide consent in a granular way, and makes it easier for them to withdraw consent if they wish. Electronic consent methods have been used to support indexing and retrieval of consent data, thus enhancing the ability to honor to patient intent and identify willing research participants. More recently, Health Sciences South Carolina ,

14874-406: The reality of consent because of social processes: a view that has started to be acknowledged in bioethics. She feels that the liberal principles of informed consent are often in opposition with autocratic medical practices such that norms values and systems of expertise often shape and individuals ability to apply choice. Patients who agree to participate in trials often do so because they feel that

15008-470: The research if they obtain a waiver of informed consent (WIC) or an emergency exception from informed consent (EFIC). The 21st Century Cures Act enacted by the 114th United States Congress in December 2016 allows researchers to waive the requirement for informed consent when clinical testing "poses no more than minimal risk" and "includes appropriate safeguards to protect the rights, safety, and welfare of

15142-451: The risk increases if a woman has experienced trauma prior to the pregnancy. Prevalence of PTSD following normal childbirth (that is, excluding stillbirth or major complications) is estimated to be between 2.8 and 5.6% at six weeks postpartum, with rates dropping to 1.5% at six months postpartum. Symptoms of PTSD are common following childbirth, with prevalence of 24–30.1% at six weeks, dropping to 13.6% at six months. Emergency childbirth

15276-443: The risk of heart disease. The organizations were criticized by bloggers and the editorial board of The New York Times , who expressed "fear that it will open the way for drug companies to bombard anxious parents with ads promoting these and other products and increase the number of parents insisting on prescriptions for their children. The ease of popping pills should not distract parents, health professionals, or policy makers from

15410-412: The risks of the procedure" and that the health benefits "are sufficient to justify access to this procedure for families choosing it and to warrant third-party payment for circumcision of male newborns", but "are not great enough to recommend routine circumcision for all male newborns". The academy takes the position that parents should make the final decision about circumcision after appropriate information

15544-498: The site. Others pointed out that this specific study is not unique but rather news organizations constantly try out different headlines using algorithms to elicit emotions and garner clicks or Facebook shares. They say this Facebook study is no different from things people already accept. Still, others say that Facebook broke the law when conducting the experiment on users that did not give informed consent. The Facebook study controversy raises numerous questions about informed consent and

15678-589: The stresses of war affect everyone involved, displaced persons have been shown to be more so than others. Challenges related to the overall psychosocial well-being of refugees are complex and individually nuanced. Refugees have reduced levels of well-being and a high rate of mental distress due to past and ongoing trauma. Groups that are particularly affected and whose needs often remain unmet are women, older people and unaccompanied minors. Post-traumatic stress and depression in refugee populations also tend to affect their educational success. Sudden, unexpected death of

15812-511: The text does not also apply this idea to disclosing information to patients. Through this text, Percival's ideas became pervasive guidelines throughout the United States as other texts were derived from them. Worthington Hooker was an American physician who in 1849 published Physician and Patient . This medical ethics book was radical demonstrating understanding of the AMA's guidelines and Percival's philosophy and soundly rejecting all directives that

15946-495: The time of the ancient Greeks . A few instances of evidence of post-traumatic illness have been argued to exist from the seventeenth and eighteenth centuries, such as the diary of Samuel Pepys , who described intrusive and distressing symptoms following the 1666 Fire of London . During the world wars , the condition was known under various terms, including ' shell shock ', 'war nerves', neurasthenia and ' combat neurosis '. The term "post-traumatic stress disorder" came into use in

16080-442: The traumatic event is also associated with a subsequent risk of developing PTSD, with experiences related to witnessed death, or witnessed or experienced torture, injury, bodily disfigurement, traumatic brain injury being highly associated with the development of PTSD. Similarly, experiences that are unexpected or in which the victim cannot escape are also associated with a high risk of developing PTSD. PTSD has been associated with

16214-594: The trial was suggested by a doctor as the best intervention. Patients may find being asked to consent within a limited time frame a burdensome intrusion on their care when it arises because a patient has to deal with a new condition. Patients involved in trials may not be fully aware of the alternative treatments, and an awareness that there is uncertainty in the best treatment can help make patients more aware of this. Corrigan notes that patients generally expect that doctors are acting exclusively in their interest in interactions and that this combined with "clinical equipose" where

16348-413: The typical case, the individual with PTSD persistently avoids either trauma-related thoughts and emotions or discussion of the traumatic event and may even have amnesia of the event ( dissociative amnesia ). However, the event is commonly relived by the individual through intrusive, recurrent recollections, dissociative episodes of reliving the trauma (" flashbacks "), and nightmares (50 to 70%). While it

16482-436: The use of medical consent forms in hospitals. However, reliance on a signed form should not undermine the basis of the doctrine in giving the patient an opportunity to weigh and respond to the risk. In one British case, a doctor performing routine surgery on a woman noticed that she had cancerous tissue in her womb. He took the initiative to remove the woman's womb; however, as she had not given informed consent for this operation,

16616-498: The vector for yellow fever transmission. His earliest experiments were probably done without formal documentation of informed consent. In later experiments he obtained support from appropriate military and administrative authorities. He then drafted what is now "one of the oldest series of extant informed consent documents." The three surviving examples are in Spanish with English translations; two have an individual's signature and one

16750-450: The very act of revealing that a study is being conducted is likely to alter the behavior studied. Author J.A. List explains the potential dilemma that can result: "if one were interested in exploring whether, and to what extent, race or gender influences the prices that buyers pay for used cars, it would be difficult to measure accurately the degree of discrimination among used car dealers who know that they are taking part in an experiment." In

16884-638: The very devices and treatments they recommend to patients. Robert Pedowitz, the former chairman of UCLA's orthopedic surgery department, reported concern that his colleague's financial conflicts of interest could negatively affect patient care or research into new treatments. In a subsequent lawsuit about whistleblower retaliation, the university provided a $ 10 million settlement to Pedowitz while acknowledging no wrongdoing. Consumer Watchdog, an oversight group, observed that University of California policies were "either inadequate or unenforced...Patients in UC hospitals deserve

17018-924: The way a person thinks and feels, and an increase in the fight-or-flight response . These symptoms last for more than a month after the event and can include triggers such as misophonia . Young children are less likely to show distress, but instead may express their memories through play . A person with PTSD is at a higher risk of suicide and intentional self-harm . Most people who experience traumatic events do not develop PTSD. People who experience interpersonal violence such as rape, other sexual assaults, being kidnapped, stalking, physical abuse by an intimate partner, and childhood abuse are more likely to develop PTSD than those who experience non- assault based trauma, such as accidents and natural disasters . Those who experience prolonged trauma, such as slavery, concentration camps, or chronic domestic abuse, may develop complex post-traumatic stress disorder (C-PTSD). C-PTSD

17152-431: The world, with more than 300 titles for consumers and over 500 titles for physicians and other healthcare professionals . These publications include electronic products, professional references, medical textbooks , practice management publications, patient education materials, and parenting books . The AAP News is the academy's official news magazine, and Pediatrics is its flagship journal. The AAP issues

17286-426: Was a student of the works of Gregory and various earlier Hippocratic physicians. Like all previous works, Percival's Medical Ethics makes no mention of soliciting for the consent of patients or respecting their decisions. Percival said that patients have a right to truth, but when the physician could provide better treatment by lying or withholding information, he advised that the physician do as he thought best. When

17420-466: Was an 18th-century United States physician who was influenced by the Age of Enlightenment cultural movement. Because of this, he advised that doctors ought to share as much information as possible with patients. He recommended that doctors educate the public and respect a patient's informed decision to accept therapy. There is no evidence that he supported seeking a consent from patients. In a lecture titled "On

17554-503: Was an electric shock to another research participant. For the study to succeed, it was necessary to deceive the participants so they believed that the subject was a peer and that their electric shocks caused the peer actual pain. Nonetheless, research involving deception prevents subjects from exercising their basic right of autonomous informed decision-making and conflicts with the ethical principle of respect for persons . The Ethical Principles of Psychologists and Code of Conduct set by

17688-419: Was developed by people who drew influence from Western tradition . Historians cite a series of medical guidelines to trace the history of informed consent in medical practice. The Hippocratic Oath , a Greek text dating to 500 B.C.E., was the first set of Western writings giving guidelines for the conduct of medical professionals. Consent by patients as well as several other, now considered fundamental issues,

17822-796: Was established to help institutionalize pediatric tobacco control activities at AAP and was named in honor of Julius B. Richmond , MD, Chair of the FAMRI Medical Advisory Board and former Surgeon General of the United States Public Health Service . The Center provides the education, training, and tools needed to protect children from tobacco and secondhand smoke. The Academy has published hundreds of policy statements, ranging from advocacy issues to practice recommendations. The academy's policy website contains all current Academy policies and clinical reports. The AAP policy regarding its statements

17956-434: Was founded in 1930 by 35 pediatricians to address pediatric healthcare standards. As of 2022 , it has 67,000 members in primary care and sub-specialist areas. Qualified pediatricians can become fellows (FAAP). The Academy runs continuing medical education (CME) programs for pediatricians and sub-specialists . The Academy is divided into 14 departments and 26 divisions. It has the largest pediatric publishing program in
